Request an Application

There are three options for obtaining a Child Support application.

1. Online - Print the application (pdf, updated 11/28/07).  Adobe Acrobat Reader is required to view and print this application. If you do not have Acrobat Reader on your computer, please visit the Adobe Web site to download the free software.
   
2. Telephone - To request an application, call the main child support number at 303-271-4300.
   
3. In person - Pick up an application at the Child Support office located at 3500 Illinois Street, Suite 1300, Golden, Co  80401. View a map.

Answering Your Questions

• Questions about non-payment or general information - Contact Child Support at 303-271-4300.
• Questions about payment - Contact the Family Support Registry at 303-299-9123. 
• When your application is in another county - Contact the Child Support Program of that county. For a list of other county, state and even international contacts, visit the state's Child Support Enforcement Web site.
