Clinic Instructions:

  • Register for a free H1N1 flu shot by filling out the appropriate Consent Form and bringing it with you. There will be forms available at site, but bringing yours already filled out, will help with flow of clinic.
  • Email us at: publichealthclinics@jeffco.us and include in email which clinic you plan on attending and which priority group you or your child fall into. This will help us estimate numbers of persons attending each of the school-located clinics.

  • Vaccinations at the clinics will be given on a first-come, first-served basis dependent upon on-site vaccine availability.  When vaccine runs out, clinic will be closed.

  • The H1N1 vaccine is administered in upper arm deltoid muscle or, in young children, the upper thigh. Wear clothing that allows access to these areas.

  • Please be prepared to wait in line. Wear appropriate shoes for standing for long periods of time and dress for inclement weather.

What to Expect: Clinic flow for Priorty Group Clinic Participants:
Upon entering the school located public health H1N1 clinic, patients will be directed to the appropriate area. Those who have pre-registered will go directly to the Screening Area. A Screener will review patient information and registration document. If needed, the patient may be referred to a Medical Screener who can respond to medical questions or issues. Once cleared by the Screener, the patient will be directed to the appropriate line to wait for an open Vaccinating Station. Once a vaccinator is available, the patient will proceed to a Vaccinating Station and receive the vaccination. From there, the patient will be asked to sit and wait for 5 minutes before exiting the clinic.

Lines may be long and stretch outside of facility. Please be prepared for weather!
