The Health Care Access Program assists people with access to health care.
For additional support with identifying and accessing community health care
services, please contact our Resource Nurse at 303-239-7029.
JCPH's Health Care Access Program:
- Assistance with Medicaid application process and documentation
- Pregnancy tests and referrals to prenatal providers
- A visit with a nurse will assist in identifying risk factors and provide immediate counseling.
- Referrals to appropriate JCDHE programs
- Pregnancy tests also available.

EPSDT Administrative Case Management and Outreach
The Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Program provides children under the age of 21 and on Medicaid with regular health checkups, immunizations and treatment for health problems. It also assists pregnant women with accessing prenatal care.
Who Benefits from EPSDT?
Any child or pregnant woman who already has a Medicaid card can get EPSDT services. If a child does not have a Medicaid card, apply at your local Social Services office or Presumptive Eligibility site.
The EPSDT Administrative Outreach Coordinator
The EPSDT Administrative Outreach Coordinator is assigned to you based on your zip code and can help keep you up to date with your child's health care. This case manager can help you find providers for well or sick care, dental needs, vision checks and eye glasses, and prenatal care for pregnant women.

CHP+ is a state insurance plan for children through 18 years of age. It is based on income eligibility. There is a small annual fee. Co-pays are based on factors such as income and family size. The Health Department does the pre-qualifying screening and the final determination is done by CHP+. There is a limit to the length of time one can be on CHP+. Kaiser Connections provides low cost health care coverage for families who otherwise could not afford health care. Connections provides full Kaiser Permanente benefits at greatly reduced rates for up to 24 months to eligible individuals and families.
